Trichotillomania is a body-focused repetitive behaviour (BFRB) characterised by the recurrent, compulsive urge to pull out one's own hair. The condition can begin at any age, though it often first appears during adolescence. It affects both males and females, and it can range in severity from mild and manageable to severe and debilitating, with significant hair loss that becomes difficult to conceal.
Hair pulling often occurs in two distinct ways. Some people engage in "focused" pulling, where they are fully aware of what they are doing and feel an intense urge or tension beforehand that is relieved by pulling. Others experience "automatic" pulling, where they pull without realising they are doing it — often while reading, watching television, studying or lying in bed. Many people experience a combination of both.
The underlying causes of trichotillomania are complex and vary from person to person. For some, hair pulling develops as a response to stress, anxiety, boredom or emotional pain. For others, there may be a sensory component — the tactile sensation of pulling or the visual inspection of the pulled hair provides a form of self-soothing or stimulation. In many cases, the behaviour becomes deeply wired into the subconscious as an automatic response to certain emotional states, situations or physical sensations.

For most people with trichotillomania, the social and emotional consequences are at least as significant as the hair loss itself. The condition creates a painful double burden: the distress of the pulling behaviour and the distress of its visible consequences.
Many people with trichotillomania become experts at concealment — carefully styling their hair to cover thin patches, using makeup to disguise missing eyebrows, wearing hats or scarves in situations where they would rather not. This constant vigilance is exhausting and isolating. Every gust of wind, every swimming invitation, every hairdresser appointment becomes a source of anxiety rather than a normal part of life.
Relationships can be profoundly affected. Many people hide their pulling from partners, friends and family, carrying the secret in silence for years. The shame and secrecy create a barrier to intimacy and genuine connection. Some people avoid relationships altogether rather than risk having their secret discovered.
